The form – sometimes incorrectly referred to as form DA 2760 – was last revised on December 1, 2002. A DD 2760 form is available below for digital download and filing or can be found on the Executive Services Directorate`s website. The DD 2760 submission process is simple, with all forms usually self-explanatory. The form contains a total of two sections: Form DD 2760 must be checked with your signature and forwarded to a captain or direct superior within ten days of receiving the form. The DD 2760 is sometimes referred to as the Lautenberg Act DD Form 2760. The form was named because of the link to the Domestic Violence Amendment (or “Lautenberg Change”) with the Gun Control Act of 1968. DD Form 2760, a firearms or ammunition possession qualification, is a form issued by the Ministry of Defence (DoD) to gather the necessary information to determine whether a serving member has been convicted of domestic violence or other crimes that would disqualify them from possession, transportation, shipment or receipt of state-issued and private firearms and ammunition. SD 2760 is also used to determine whether reassignment, reclassification or other administrative action is warranted against the service member who declares the form. Under this law, no person convicted of a domestic violence offence has the right to receive, possess, ship or transport ammunition or firearms. The law also provides for the sale or disposal of firearms and ammunition to persons known or reasonably convicted of domestic violence.
